To make an appt. call 1-800-234-8888 AFTER WEDNESDAY, October 8. Operators are available Monday through Friday between 8:30 am and 5:00 pm. Please provide your Physician name and address at the time of the call. You must bring a prescription from your physician to your appointment! Note: This screening is not covered by Medicare and most other insurance companies. Check or money order only will be accepted the day of the program and should be made payable to AMBA Wellness Programs. An additional $5.00 sponsor group fee is payable to Rotary Club of Meadville AM. Photo ID required at check-in
